A Short Pediatric Care Survey
Feb 6, 2024
By Lopez Community For Health Care
It's always children's month on Lopez and here's a survey every parent should consider.
This one, even shorter than our county’s excellent survey about your child day care needs, concerns your child’s health, or pediatric services you would like to see on Lopez.
The on line survey is entirely confidential with summarized results to be posted and sent to our Hospital District which has expressed a keen interest.
Please take just few minutes to complete this important survey by clicking on this link:

The Lopez Community for Health Care would like to thank the Family Resource Center for their work in preparing this survey. You may notice there is no Spanish translation being offered here. That is because a Spanish language version has been posted on the Hispanic community’s Facebook page for several weeks. It was created as a collaboration between LCHC and five LIFRC Community Health Workers known as the Promotoras. The survey here is substantially the same.
